Certificated of non-competitive sports medical fitness
Photocopy of parent's and minor's identity document

- ​authorizes the participation of the son/daughter in the indicated sports activity;
- declares that the minor has a medical certificate that is currently valid;
- relieves the organization of any civil or criminal liability for potential accidents that occur during the activity, except in cases of intentional or serious fault;
- authorizes furthermore, in case of necessity, the transport and medical treatment of the minor, according to the instructions of the health personnel.

Privacy

​ Personal data will be processed in compliance with EU Regulation 2016/679 (GDPR) and used exclusively for organizational and managerial purposes related to sports activities.
